The Practice Boundary

Our practice boundary is the River Don to the north, which then runs down King Street to Union Street. It extends down Great Western Road, stopping just before Cults. It joins back to the River Don by including Hazelhead, Sheddocksley and Heathryfold on its western boundary. How to register

If you are looking to register as a patient with us please visit Elmbank Group Practice reception to fill out the forms.

You will need to bring physical copies of Proof of Identification and Proof of Address.

For Proof of Identification we accept:

• Birth Certificate
• Student ID card
• Driving Licence
• Passport or HC2 certificate
• Home Office app registration card

For Proof of Address we accept:

• Amenities bills
• Tenancy agreements
• Bank Statements

You must be in our catchment area to be registered as a patient with us. Please check with reception if your address is within our area before registering.
